How do I determine if Blinkeze External Lid Weight is the right treatment for me?
Patient Ordering Process:
- Blinkeze External Lid Weights require a doctor’s prescription
- Contact your doctor to discuss whether Blinkeze is the appropriate treatment for your condition.
- Your doctor may determine Blinkeze is the appropriate treatment for you or you may be referred to a specialist such as an Ophthalmologist or Otolaryngologist. An Ophthalmologist is a medical doctor specializing in eye and vision care. An Otolaryngologist is a medical doctor specializing in conditions of the ear, nose and throat.

- To prescribe a Blinkeze External Lid Weight for you, your doctor will need to complete the BLINKEZE PHYSICIAN ORDERING FORM and fax the form along with a PRESCRIPTION to MedDev Corporation at (408) 730-9732.
- If your doctor authorizes you to order your Blinkeze Weight directly from MedDev Corporation you will need to call MedDev Customer Service at 800-543-2789, 8:30am to 5pm M-F (PT). MedDev Customer Service will confirm receipt of your prescription and verify your shipping information. Pre-payment by credit card or mailing a personal check in advance of shipment payable to ‘MedDev Corporation’ is required. Blinkeze External Lid Weights are shipped FedEx Ground (5-7 business days). Should you need your Blinkeze sooner, MedDev can arrange for next day or overnight delivery. Additional shipping charges will apply.
- Your Blinkeze External Lid Weight treatment kit includes your prescribed weight in the appropriate skin tone, EyeClose® double-sided adhesive strips (100 strips) for attaching the weight to your eyelid (approximate 3-month supply), a storage box with mirror for attaching the weight while traveling and instructions for use. Additional adhesive strips (100 strips) can be ordered at the time of your initial order to save future shipping costs.
- There is no specific code for obtaining reimbursement from your insurance company for the Blinkeze External Lid Weights as a treatment for eyelid closure deficiency (lagophthalmos). In some cases an insurance company will cover part or all of the cost of the External Lid Weight, but coverage cannot be guaranteed in all circumstances. Because of this, MedDev has maintained the lowest possible “out-of-pocket” cost for the device. MedDev can provide your doctor upon request a Physician Billing Guide with information on submitting a claim to private insurance for potential coverage of your Blinkeze External Lid Weight.
- Upon receipt of your Blinkeze External Lid Weight treatment kit read the Patient Instructions for Use (IFU) insert carefully before beginning your treatment.
- Your Blinkeze weight will help close your affected eyelid when your levator (blink reflex) muscle relaxes. Your doctor may also prescribe use of ophthalmic ointments or drops in conjunction with your Blinkeze treatment.
- Contact your doctor if you are unable to close your eyelid while wearing your Blinkeze External Lid Weight. You may need to be re-fit for a heavier wight. Alternatively, if your Blinkeze weight is too heavy and you are unable to open your eyelid or your Blinkeze weight obscures your vision, you will need to be re-fitted for a lighter weight.
- MedDev return policy permits returns within the first 30 days from purchase for a different size weight in the same skin tone. An RGA number is required for the return and can be obtained by calling MedDev Customer Service at 800-543-2789, 8:30am to 5pm M-F (PT).
- Your doctor will need to fax a new prescription to MedDev Customer Service (Fax# 408-730-9732)

How do I prescribe a Blinkeze External Lid Weight for my patient?
Physician Ordering Process:
- Blinkeze External Lid Weights are available in a range of sizes from 0.6 g to 2.8 g by 0.2 g increments. The Blinkeze External Lid Weight Treatment Kit includes the prescribed weight in the appropriate skin tone, EyeClose® double-sided adhesive strips (100 strips) for attaching the weight to the eyelid (approximate 3-month supply), a storage box with mirror for attaching the weight while traveling and instructions for use. Additional adhesive strips (100 strips/box) can be ordered.
- MedDev Tantalum Eyelid Weight Sizing Set is used to determine the appropriate Blinkeze weight size for your patient. Eyelid sizing weights range in sizes from 0.6 g to 2.8 g by 0.2 g increments and are attached to the upper lid in either ascending or descending weight order to determine the weight that adequately closes the eyelid upon relaxation of the levator muscle. Typical over correction of 0.2 g is suggested as the levator strengthens over time. Alternately, MedDev survey indicates Blinkeze 1.4 g to be the most universal weight size prescribed over a 5-year period.
- Blinkeze External Lid Weights are available in five eyelid skin tone shades. A skin tone ruler that is included with the Tantalum Eyelid Weight Sizing Set or can be obtained by calling MedDev Customer Service permits selection of the appropriate skin tone shade that best matches the patient’s complexion. A suffix of -0 for ‘light’; -1 for ‘pink/neutral’; -2 for ‘tan’; -3 for ‘medium brown’; and -4 for ‘dark brown’ is added to the end of the catalog number.
- The completed BLINKEZE PHYSICIAN ORDERING FORM (download here) and patient prescription can be faxed to MedDev Customer Service at fax # (408) 730-9732. The form authorizes the patient to call MedDev directly to order the prescribed Blinkeze External Lid Weight. Alternatively, the weight can be ordered by the physician and the patient ‘fitted’ in-office.
- Blinkeze External Lid Weights are contraindicated for patients with excessively wrinkled or sagging eyelid skin (dermatochalasis) or with a known hypersensitivity to tantalum or adhesive.
- Should the weight not sufficiently close the eyelid or over correct, MedDev permits returns for a different size weight in the same skin tone within the first 30 days. A new prescription is required.
- An RGA number is required for the return and can be obtained by calling MedDev Customer Service at 800-543-2789, 8:30am to 5pm M-F (PT).
- Patients with prolonged paralysis may require reevaluation and possible replacement of a Blinkeze External Lid Weight with a surgically implanted gold or platinum weight.
